Information about Ontario's vaccine rollout is changing rapidly. This page will be updated as more information becomes available. Read below to find out if you are eligible to book an appointment.

Some vaccine sites may be prioritizing certain eligible groups over others. If you cannot book an appointment for your eligibility group using the links in Step 1, visit the individual links below to see which vaccination sites are taking bookings for your group.

UHN Vaccine Registry

  • Patients who meet any of the UHN's priority group criteria may pre-register as Dr Facy's clinic is located within the mid-west Toronto catchment area.

Other Hospital Sites & Ontario Health Team Community Clinics

  • Check individual sites to see which individual groups are currently being vaccinated.

  • Note that some sites may be restricted to your home catchment area.

Ontario Pharmacies

  • Currently offering first doses of the Moderna vaccine.

City of Toronto Immunization Clinics

  • Bookings for individuals eligible for vaccines through Ontario's provincial booking system.
